Students fill out four short questionnaires without putting their names on them. They hand the papers in and the papers (questionnaires) are shuffled/mixed. The teacher then hands out these slips of paper at random (or students can draw them from a box or bowl) so that each student has four questionnaires, none of which are his/her own. Then, in a mixer format, students must ask others the questions on their questionnaires until they find the student that gives the same answers that are on the questionnaire given them by the teacher.
